Pankreaskarzinom: Chirurgische Konzepte zur kompletten Tumorentfernung

Zusammenfassung

Die Pankreasresektion hat sich in den letzten Jahren zu einem standardisierten Eingriff mit niedriger Morbidität und Letalität gewandelt. Der erweiterte chirurgische Zugang mit venöser Gefäßresektion und/oder multiviszeralen Resektion ist anzustreben, wenn keine signifikanten Kontraindikationen bestehen. Die arterielle Resektion kann in selektionierten Fällen gerechtfertigt sein, ist aber nicht generell zu empfehlen. Die chirurgische Exploration und Resektion eines lokal wieder aufgetretenen Tumors sollte, wenn technisch möglich, in Betracht gezogen werden, da es die Prognose verbessert. Die erweiterte Lymphadenektomie hat sich als onkologisch nicht gerechtfertigt erwiesen und ist deshalb nicht indiziert. Der chirurgische Zugang sollte jedoch immer Teil eines interdisziplinären multimodalen Behandlungskonzepts sein, um damit die besten Ergebnisse für die Patienten zu ermöglichen.
