Zusammenfassung

Der Clusterkopfschmerz wird als extrem stark erlebt und ist mit sehr hohem Leidensdruck verbunden. Es handelt sich um einseitige Kopfschmerzen, die mit Tränen, Augenrötung, nasaler Kongestion, Rhinorrhoe, Ptosis, Rötung oder Schwitzen im Gesicht einhergehen und in der Regel episodisch über einige Wochen hindurch und in zirkadianer Rhythmik auftreten. Zur Attackentherapie können neben 100 % Sauerstoff über Maske die Triptane Zolmitriptan als Nasenspray oder Sumatriptan subkutan eingesetzt werden. Kurzzeitig können zur Prophylaxe Steroide eingesetzt werden. Für eine längere Prophylaxe ist Verapamil das Mittel der Wahl. In therapieresistenten Fällen kann eine Neurostimulation, wie z. B. die Stimulation des Ganglion sphenopalatinum, erwogen werden. In Studien werden derzeit zur Prophylaxe monoklonale Antikörper gegen das CGRP („calcitonin gene-related peptide“) erprobt.
