Neue Substanzen in der medikamentösen Therapie des metastasierten Pankreaskarzinoms – Was ist Standard nach Leitlinie für wen, welche Therapiesequenz, was bringt die Zukunft?

Zusammenfassung

In letzter Zeit konnte eine deutliche Verbesserung in der Therapie des metastasierten Pankreaskarzinom (PC) erreicht werden. In der Erstlinientherapie haben sich neben der Standardchemotherapie mit Gemcitabin neue Therapiekombinationen mit Nab-Paclitaxel/Gemcitabin und FOLFIRINOX (Fluoruracil, Folinsäure, Irinotecan, Oxaliplatin) etabliert. Die Indikation für eine Zweitlinientherapie nach Progression ergibt sich aus dem Eastern-Cooperative-Oncology-Group Performance Status (ECOG-Performance-Status), klinischen Parametern (Bilirubin) und der Art der Vorbehandlung in der Erstlinie. Seit dem Jahr 2016 hat sich mit nanoliposomalem Irinotecan (nal-IRI) plus 5‑FU/LV (5-Fluoruracil/Leucovorin) eine neue Substanz etabliert. Neue Therapieansätze bei Patienten mit Keimbahnmutationen (häufig BRCA1, BRCA2, PALB2 und die ATM-Gene) und immunologische Therapieansätze mit sog. Checkpointinhibitoren werden in rezenten Studien untersucht. Die Daten zum stromalen Hyaluronsäuregehalt machen Hoffnung auf einen klinisch relevanten Biomarker zur Therapiesteuerung beim Pankreaskarzinom. Im Rahmen der personalisierten Therapie mittels „whole genome sequencing“ (WGS) und RNA-Sequenzierung (RNA-Seq) können molekulare Marker gemessen werden. Die palliative Therapie fokussiert sich auf Symptomenkontrolle, Ernährung, Enzym‑, Diabetes- und Vitamintherapie, Schmerztherapie und den psychoonkologischen Support.
