Lipoprotein(a) als wichtiger Risikofaktor für Gefäßverschlüsse. Bericht über eine beidseitige nicht-entzündliche anteriore ischämische Optikusneuropathie mit erfolgreicher Behandlung eines Auges und Langzeitbeobachtung

Zusammenfassung

Hintergrund

Bisher gibt es keine sichere erfolgversprechende („evidence-based“) Behandlung bei einer nicht-arteriitischen anterioren ischämischen Optikusneuropathie (N-AION).

Patient und Methode

Eine 59-jährige Patientin bemerkte 1997 eine akute Sehminderung rechts infolge einer nicht-entzündlichen anterioren ischämischen Optikusneuropathie (N-AION). Trotz Therapie mit isovolämischer Hämodilution, Kortikosteroiden und Aspirin verschlechterte sich das Sehvermögen schubweise bis auf Erkennen von Handbewegungen und später auf Lichtscheinwahrnehmung. Drei Jahre später entstand auch links akut eine Sehminderung. Der Visus war auf 0,8 reduziert, es bestand ein unvollständiger Gesichtsfeldausfall nach unten infolge einer N-AION.

Ergebnis

Nach der Visusminderung des linken Auges wurde ein erhöhter Lipoprotein (a)-Spiegel auf 89 mg/dl (normal ≤ 30 mg/dl) sowie ein leicht erhöhter Homocystein-Spiegel auf 13,47 µmol/l im Serum festgestellt.
Es erfolgte eine umgehende H.E.L.P.-Behandlung (Heparin-induzierte extracorporale LDL/Fibrinogen Präzipitationsbehandlung), die in kurzen Abständen von mehreren Tagen dreimal durchgeführt wurde. Hierunter senkte sich der Lipoprotein (a)-Spiegel auf normale Werte. Danach hatte sich das Sehvermögen des linken Auges nicht verschlechtert. Anschließend wurde die Patientin auf Phenprocoumon (Marcumar®) eingestellt. Sie ist laufend mit diesem Medikament weiterbehandelt worden. Der Gesichtsfeldbefund ist im Laufe von 16 Jahren gleich geblieben. Der Lipoprotein (a)-Spiegel war bei späteren Kontrolluntersuchungen wieder pathologisch erhöht.

Schlussfolgerung

Bei einer akuten Sehminderung durch eine ischämische Optikusneuropathie ist eine H.E.L.P.-Behandlung zu empfehlen, insbesondere bei erhöhtem Lipoproteingehalt des Blutes. Entscheidend ist der frühzeitige Behandlungsbeginn. Zur Langzeittherapie eines erhöhten Lipoprotein (a)-Spiegels ist auch wegen der Gefahr von Gefäßverschlüssen anderer Organe (Herzinfarkt, Schlaganfall) eine Phenprocoumonbehandlung bzw. eine Behandlung mit analogen Medikamenten erforderlich.
