CLINICAL PRESENTATION
A 29-year-old woman was admitted to hospital with chest pain and joint pain. Electrocardiography revealed ST-segment depression in leads V4 to V6. Coronary arteriogram demonstrated numerous aneurysms involving both the epicardial and intramyocardial segments of the left anterior descending, circumflex and right coronary arteries, and the right coronary artery was occluded (Figure 1). PAN-associated aneurysmal disease can be present in up to 94% of patients [104], and has been described in the cerebral, coronary, celiac, hepatic, and renal arteries [104–108], but no reports of aortic involvement or diffuse disease across multiple territories exist. One of the most characteristic findings on angiography is multiple saccular aneurysms resulting from pan-necrosis of the internal elastic lamina of the medium and small arteries, tending to occur at branching points of affected arteries, with a predisposition for the kidneys and liver [104].
