Zusammenfassung

Von 1977-1983 wurden 64 Patienten mit teils komplizierten Harnröhrenstrikturen mit der Meshgraft-Urethroplastik behandelt. Das Prinzip der Operation ist die freie Transplantation genetzter Vollhaut bzw. Spalthaut. Die Operation wird zweizeitig durchgeführt. Offene und trockene Transplantatbehandlung garantieren ein sicheres und problemloses Einheilen des Transplantates. So werden ideale Voraussetzungen für die spätere Rekonstruktionder Harnröhre geschaffen.

Die Operation wurde in den Jahren von 1977 bis 1983 in 63 von 64 Fällen erfolgreich durchgeführt. Sie ist grundsätzlich zur Behandlung aller Formen der Harnröhrenstriktur geeignet. Die wichtigsten Indikationen sind: komplizierte Harnröhrenstrikturen, wie außergewöhnlich langstreckige Strikturen, Rezidivstrikturen, auch nach mehrfacher Rezidivoperation, infizierte Strikturen und Strikturen bei querschnittsgelähmten Patienten.

Abstract

From 1977 to December 1983 a total of 64 patients with posterior, anterior or extended urethral stricture have been treated by using free foreskin-grafts after the mesh procedure of the transplant. This operation ist completed as a two-stage-procedure. Of paramount importance for success is the open and dry treatment of the graft which assures an ideal situation for the later reconstruction. Healing without formation of fibrous tissue is the rule. In 63 patients this method was applied with an excellent anatomical and functional result. The most important indications are recurrent strictures after several operations, extended strictures of the whole length of the urethra, strictures with chronic inflammations, as well as problematic strictures in paraplegics.
