Partizipative Entscheidungsfindung und ihre Umsetzung im Gesundheitswesen - Vom Patienten gewünscht, von der Politik gefordert

Eine Patientenbeteiligung wird von mündigen Patienten immer mehr gewünscht und zu Recht eingefordert. Ziel ist es, die Patienten als gleichberechtigte Partner in den medizinischen Entscheidungsprozess mit einzubeziehen. Der Bedarf ist groß: Derzeit fühlen sich in Deutschland nur knapp die Hälfte der Patienten ausreichend an der Therapieentscheidung beteiligt. Erfahrungen aus Modellprojekten zeigen, dass die Einbeziehung der Patienten im Sinne der partizipativen Entscheidungsfindung von Ärzten in Trainings erlernt werden kann, was zu mehr Zufriedenheit von Patienten und Ärzten sowie zu besseren Behandlungsergebnissen führt. Ärzte und Medizinstudenten sollten somit in Fort- und Weiterbildungen mit den neuen Rollen und Erwartungen der Patienten vertraut gemacht werden. Ärzte verstehen die Zeichen der Zeit und sind gewillt, sich auf ein moderneres Rollenverständnis einzulassen, wie das rege ärztliche Interesse an angebotenen PEF-Kommunikationstrainings zeigt. Trotzdem ist die partizipative Entscheidungsfindung noch nicht flächendeckend in der Praxis angekommen.

Increasingly, responsible and informed patients wish to be involved in the decision-making process, and they are right to do so. The goal is to integrate the patient on an equal footing with the physician - the demand is certainly there: Currently, only just under one-half of the patients in Germany consider themselves to be sufficiently involved in decisions regarding their treatment. Experience gained from model projects shows that patient involvement in the form of shared decision making can be learned by physicians in training, and leads to greater satisfaction on the part of both patient and doctor, as well as to better treatment. This suggests that, in continued and further education programs, doctors and medical students should be familiarized with this new role and the patients' expectations. Physicians recognize the signs of the times and are prepared and willing to accept and adopt a more modern role vis-à-vis their patients, as is shown by their lively interest in the training programs for shared decision making now being offered. It must, however, be noted that shared decision making has not yet been adopted on a nationwide basis.
