Anaphylaktischer Schock bei einem 25-jährigen Asylbewerber aus Georgien

Anamnese und klinischer Befund: Ein 25-jähriger Asylbewerber aus Georgien kam zur Aufnahme mit akuten Schmerzen im rechten Oberbauch, Schwindel, Luftnot, generalisierter Urtikaria und positivem Schockindex (RR 80/40 mm HG, Herzfrequenz 120/min) mehrere Minuten nach Verzehr einer Tütensuppe. Unter dem Verdacht auf eine Nahrungsmittelallergie gab der Notarzt Prednisolon, H1- und H2-Antihistaminika und Elektrolytlösung intravenös.

Untersuchungen: Im Notfall-Labor fanden sich lediglich eine geringe Erhöhung der eosinophilen Leukozyten sowie der Aktivitäten der GOT und GPT im Serum. In der abdominellen Sonographie und Computertomographie zeigten sich eine multivesikuläre, septierte zystische Raumforderung im rechten Leberlappen (Segment VII, 13  ×  9  × 8 cm) und perihepatische Flüssigkeit.

Therapie und Verlauf: Unter dem Verdacht einer rupturierten Echinokokkus-Zyste erfolgte die Notfall-Laparotomie mit Endozystektomie. Anschließend wurde eine anthelmintische Therapie mit Albendazol über 3 Monate durchgeführt. In den Verlaufskontrollen bis zu einem Jahr nach der Operation war der Patient beschwerdefrei. Sonographisch und computertomographisch zeigte sich lediglich ein kleiner Residualsubstanzdefekt im rechten Leberlappen.

Folgerung: Bei einer anaphylaktischen Reaktion sollte bei Patienten aus Echinokokkose-Endemiegebieten an die Möglichkeit einer rupturierten Echinococcus granulosus-Zyste gedacht werden. Abdomensonographie und Serologie (cave: substantielle Rate falsch-negativer Resultate!) sind wegweisende Untersuchungen.

History and admission findings: A 25-year-old asylum seeker from Georgia was admitted to hospital with acute right upper quadrant abdominal pain, vertigo, dyspnea, generalized urticaria and positive shock index (blood pressure 80/40 mmHg, pulse 120/min) several minutes after eating an instant soup.

Investigations: Laboratory investigations showed a slight increase of the eosinophilic cell count and GOT and GPT activities. Abdominal ultrasound scan (USS) and computed tomography (CT) revealed a multivesicular septated cystic space-occupying lesion of the right liver lobe (segment VII, 13 × 9 × 8 cm) and perihepatic fluid.

Treatment and course: Intravenous steroids, H1 and H2 antagonists and fluid were given. Emergency laparatomy with endocystectomy was performed. A 3-month course of antihelmintic therapy with albendazole was applied. During follow-up up to one year after surgery the patient did well. Ultrasonography and computed tomography showed only a small residual defect in the right liver lobe where the cyst had been removed.

Conclusion: In patients from echinococcosis-endemic regions who develop an anaphylactic reaction, a ruptured Echinococcus granulosus cyst should be considered in the differential diagnosis. Abdominal ultrasonography and serology (to be noted: substantial rate of false negative results!) should be performed.
