Zusammenfassung

Hintergrund Ulzerationen der Hornhaut stellen Prozesse dar, die in schweren Fällen nur mittels operativer Engriffe erfolgreich behandelt werden können. Die UV-Bestrahlung mit Riboflavin ist ein neues Verfahren, bei dem solche Einschmelzungsprozesse aufgehalten werden können.

Patienten und Methode Es wurden vier Patienten mit einschmelzenden Ulzera der Hornhaut unterschiedlicher Genese behandelt. Die UV-A-Bestrahlung erfolgte einmalig. Es wurden zwei UV-Leuchtdioden, die eine Wellenlänge von 370 nm emittierten mit einer Bestrahlungsstärke von 2,5 mW/cm2 und einer Bestrahlungsdauer von 30 Minuten verwendet. Die mit 2 - 3 Tropfen 0,11 %-iger Riboflavinlösung betropfte Hornhaut wurde im Abstand von etwa 1 cm Entferung bestrahlt, so dass sich eine bestrahlte Fläche mit einem Durchmesser von 6 - 7 mm ergab.

Ergebnisse Bei drei von vier Patienten kam es nach der UV-Vernetzung zum Sistieren des Einschmelzungsprozesses, was ein operatives Vorgehen mindestens zeitweilig erübrigte.

Schlussfolgerungen Insgesamt kann das oben beschriebene Verfahren in desperaten Fällen, wegen des Fehlens von Nebenwirkungen, zumindest als Behandlungsversuch vor eventuellen späteren invasiven Eingriffen empfohlen werden.

Background Corneal melting is a serious condition and in many cases maybe managed only by surgery. UV-radiation with Riboflavin is a new method that may stop the melting process of the cornea.

Patients and method Four patients suffering from melting ulcera of the cornea of various origin underwent a single UV-radiation. Two ultraviolet diodes were used emitting light with an wavelength of 370 nm with an energy of 2,5 mW/cm2. The radiation time was 30 minutes. Before he radiation was performed 2 - 3 drops of 0,11 % Riboflavinsolution were applied to the cornea. The treatment area was 6 - 7 mm in diameter.

Results After the treatment in three of the four patients the melting process of the cornea stopped. At least temporarily, a surgical procedure could be delayed.

Conclusion Because of the absence of any side-effects in serious cases the method is recommended prior to surgery.
