Alkohol- und Benzodiazepinkonsum bei älteren und hochbetagten Menschen – Ergebnisse aus dem INTERREG-Projekt Alter und Sucht[*]

Zusammenfassung
Fragestellung: Bisher liegen wenig Studien, die Selbstsangaben, Fremdangaben und objektive Biomarker hinsichtlich des Konsum von Alkohol, und Benzodiazepinen bei älteren Menschen kombinieren, vor. Ziel war es, im Rahmen eines EU geförderten INTERREG-Projektes die IST Situation im Projektgebiet (Bundesland Salzburg, Landkreis Bad Reichenhall und Landkreis Traunstein) in Krankenhäusern und Seniorenheimen zu erheben. Hier wurden neben Fremd- und Selbstauskunft auch erstmalig objektive Biomarker wie Ethylglukuronid in Urin und Haaren sowie eine Laboranalyse mittel Flüssigkeitschromatografie auf Benzodiazepine eingesetzt.
Methode: Im Projektgebiet wurden 131 Hausärzte/Internisten, 455 Seniorenheimbewohner in 33 Seniorenheimen (SH) und 123 Patienten in 4 Klinken (KH) befragt. Zusätzlich konnten Fremdauskünfte für die Patienten der Kliniken und die Seniorenheimbewohner bei Stationsärzten, Stationspflegern und Pflegedienstleitern eingeholt werden. Den Patienten in den Krankenhäusern und den Seniorenheimbewohnern (SHB) wurden zusätzlich Urin und Haare abgenommen, um den Konsum von Alkohol und Benzodiazepinen zu objektivieren. Die Bestimmung erfolgte mittels chromatografischer Methoden.
Ergebnisse: In den Seniorenheimen wurden 304 Urin- und 334 Haarproben auf Ethylglukuronid und 301 auf Benzodiazepine untersucht. Bei Krankenhauspatienten wurden 82 Haarproben und 63 Urinproben analysiert.
a) Die Suchtproblematik wird im Krankenhaus mit 1/3 am höchsten eingeschätzt, gefolgt von den Hausärzten und Internisten (jeder 10te) und Seniorenheimen (rund 7%).
b) Die Ergebnisse aus den Haaranalysen zeigen, dass rund 10% der Seniorenheimbewohner und rund 30% der Patienten in Krankenhäusern über den Empfehlungen von 10 g Alkohol pro Tag trinken. Weiter konsumieren 34,2% der Seniorenheimbewohner und 28,6% der Patienten in Krankenhäusern Benzodiazepine.
c) Bei 60% der exzessiv Alkohol konsumierenden Seniorenheimbewohnern geht die Pflege von Abstinenz oder moderatem Konsum aus.
d) Bei 68% der Benzodiazepin positiven Seniorenheimbewohnern geht die Pflege davon aus, dass keine Benzodiazepine verabreicht werden.
e) 1/3 der Bewohner sehen den Allgemeinmediziner 14-tägig. Rund 85% der Bewohner haben keine psychiatrische Versorgung.
Schlussfolgerung : Suchtprobleme im Alter sind keine Seltenheit. Eine Sensibilisierung von Betroffenen, Ärzten, Pflegenden und Angehörigen ist wichtig, um diese Probleme nicht zu übersehen. Es müssen aber auch erst Verfahren verbessert und entwickelt werden, die Suchtprobleme im Alter geeignet erheben und altersspezifische Therapie- und Behandlungsprogramme angeboten werden.

Abstract
Background: There are few studies about the prevalence of alcohol and benzodiazepine use among elderly employing besides self reports and third party reports also objective biomarkers. Therefore an EU funded INTERREG study was conducted in the county of Salzburg and 2 rural districts of Bavaria, Germany.
Aim: The aim of this study was to assess the alcohol consumption and the use of benzodiazepine among elderly in different settings using self reports and questionnaires, third party reports and biomarkers.
Method: 131 General Practitioners (GP) or internal specialists, 455 elderly in 33 nursing homes (NH) and 123 inpatients of 4 hospitals participated in this study. Additionally, third party reports were collected from the ward physicians, nurses and nursing managers for inpatients and elderly in nursing homes. In the nursing homes hair and urine was collected to analyze alcohol and benzodiazepine use by using chromatographic methods.
Results: a) The estimation of substance use disorders is with 30% in hospitals very high, followed by the estimation of GPs/internal specialists with 10% for patients over 60 years and 7% in nursing homes. b) According to the hair analysis almost 10% of elderly in nursing homes almost 30% of inpatients of hospitals drink above the recommendation of the National Institute for Alcohol Abuse and Alcoholism which is 10 g ethanol per day. With regard to the benzodiazepine use employing chromatographic methods for urine analysis 34.2% of the urine samples of elderly in nursing homes and 28.6% of inpatients in hospitals were benzodiazepine positive. c) Of 60% of elderly in nursing homes who consume alcohol at an excessive level (>60 g /day) the nursing staff assumed that they are sober or drink moderately. d) Of 68% of elderly in nursing homes who use benzodiazepine according to the laboratory analyses the nursing staff did not know about the use.
Conclusion: Addiction problems among elderly are an important aspect as quality of life, physical and mental health may be influenced. Therefore the awareness must be increased among elderly, caregivers and physicians to diagnose the substance use disorder and to offer and facilitate treatment.
