Introduction of a New Standardized Assessment Score of Spine Morphology in Osteogenesis Imperfecta

Abstract

Purpose: Osteogenesis imperfecta (OI) is a rare hereditary disease leading to multiple bone deformities and fractures. In the absence of causal therapy, a symptomatic approach is based on treatment with bisphosphonates and physiotherapy. The clinical and radiological manifestations vary. Therefore, standardization and quantification for an objective comparison, especially during therapy, are required. In this paper, radiological changes of the spine are quantified according to their clinical relevance to define a scoring system that transfers the morphological changes into a single value representing the severity of the disease.

Materials and Methods: 268 lateral spine X-rays of 95 patients with OI (median age 5.6 years) were assessed. The findings were classified based on their clinical relevance.

Results: The three criteria, vertebral compression, thoracolumbar kyphosis and deformity type, were quantified in a new grading system. Based on this, a “severity classification” (1 to 5) was defined with implications for diagnostics and treatment. A mathematical formula that takes into account the three criteria and their correlations to clinical relevance, resulting in a “severity score”, was developed.

Conclusion: “Severity classification” and “severity score” introduce a new concept for a standardized evaluation of spine X-rays in patients with OI. For both scientific and routine purposes, it provides the user with a simple and easy-to-handle tool for assessing and comparing different stages of severity prior to and during therapy with detailed accuracy.

Zusammenfassung

Ziel: Die Osteogenesis imperfecta (OI) ist eine seltene Erkrankung, bei der es zu Knochendeformierungen und Frakturen kommt. Bisphosphonate stellen neben physiotherapeutischen Maßnahmen eine entscheidende Säule der Therapie dar. Klinische und radiologische Ausprägung variieren stark, sodass eine Standardisierung und Quantifizierung zur besseren Vergleichbarkeit insbesondere unter Therapie anzustreben ist. In der vorliegenden Arbeit werden die radiologischen Veränderungen der Wirbelsäule entsprechend ihrer klinischen Relevanz quantifiziert und hieraus ein Score-System entwickelt, das die morphologischen Kriterien in einen einzigen, den Schweregrad der Erkrankung widerspiegelnden Zahlenwert überführt.

Material und Methoden: 268 Röntgenaufnahmen der Wirbelsäule im seitlichen Strahlengang von insgesamt 95 Patienten mit OI (Median 5,6 Jahre) werden nach 3 Kriterien – Grad der Wirbelkörperkompression, Winkel der thorakolumbalen Kyphose und Typ der Deformierung – ausgewertet. Die Ergebnisse werden basierend auf ihrer klinischen Relevanz quantifiziert.

Ergebnisse: Basierend auf der genannten Quantifizierung wird eine „Schweregrad-Klassifikation“ (1 – 5) definiert mit Implikationen für Diagnostik und Therapie. Eine mathematische Formel, die die 3 Kriterien und ihre Korrelationen für die klinische Relevanz umsetzt, führt zu einem detaillierten „Schweregrad-Score“ (1 – 138). Dessen Punktewerte erlauben, in einer Tabelle zusammengefasst, Röntgenbefunde der Wirbelsäule bei OI einfach und objektiv zu quantifizieren.

Schlussfolgerung: „Schweregrad-Klassifikation“ und „Schweregrad-Score“ führen ein neues Konzept für eine standardisierte Evaluation von Röntgenbildern der Wirbelsäule bei Patienten mit OI ein. Sowohl für wissenschaftliche Auswertungen als auch die klinische Routine bietet es eine einfach anzuwendende Methode, um den Schweregrad der morphologischen Veränderungen vor und unter Therapie zu beurteilen, und erlaubt objektivierbare interindividuelle Vergleiche.
