Herausforderungen der digitalen Medizin

Zusammenfassung. Die Digitalisierung erfasst zunehmend mehr Teilbereiche, auch im Gesundheitswesen. Um den medizinischen Betrieb 365 × 24 Stunden sicherzustellen, sind immer mehr personelle und finanzielle Mittel erforderlich. Die Transformation der Papierkrankengeschichten in elektronische Patientendossiers sollte zunächst primär der Dokumentation dienen. Heute werden Klinikinformationssysteme zunehmend auch als Plattform zur Kommunikation aller am Patientenprozess beteiligten Personen verwendet – in der Schweiz jedoch meist noch ohne direkte Informationsmöglichkeiten für Patienten. Digitalisierte Prozesse werden zur Effizienzsteigerung eingesetzt, werden jedoch vermehrt auch für klinische und administrative Entscheidungsunterstützung und Qualitätssicherung genutzt. Von der Einführung des elektronischen Patientendossiers in der Schweiz 2020 verspricht man sich eine betriebsübergreifende, lückenlosere Dokumentation der Patientenbetreuung. Davon dürften insbesondere auch multimorbide Patienten profitieren, die oft in verschiedenen Institutionen und von unterschiedlichen Fachspezialisten betreut werden. Fortschritte in der künstlichen Intelligenz eröffnen auch in der Medizin Chancen. Neben der Gewährleistung des zuverlässigen Datenschutzes ist die unzureichende Interoperabilität der eingesetzten Systeme eine Herausforderung. Semantisch strukturierter, maschinenlesbarer Datenaustausch ist für eine vernetzte Dienstleistung und international kompetitive Forschung notwendig.


Challenges of Digital Medicine

Abstract. Digitization is increasingly covering more and more sectors, including medicine. To ensure medical operation 365 × 24 hours, progressively more human and financial resources are necessary. The transformation of patient histories from paper into electronic patient records focused initially on documentation. Today, hospital information systems are increasingly used as a platform for the communication of all professionals involved in the patient process – in Switzerland, however, so far without providing patients direct access to their data. Digititizing processes intend to increase efficiency, but also to enhance clinical and administrative decision support and quality assurance. The introduction of the electronic patient record in Switzerland in 2020 is expected to provide cross-company, more complete documentation of patient care. Multimorbid patients, often treated in different institutions and by different specialists, should benefit from this in particular. Advances in artificial intelligence offer new opportunities in medicine. Challenges include ensuring reliable data protection, and better interoperability of the systems involved. Semantically structured, machine-readable data exchange is a necessity for both networked services and internationally competitive research.


Résumé. La numérisation couvre de plus en plus de domaines, y compris la médecine. Afin d’assurer le domaine médical 365 x 24 heures, de plus en plus de ressources humaines et financières sont nécessaires. Le passage du dossier du patient du papier à une version électronique avait pour but premier de documenter. Aujourd’hui les systèmes informatiques des hôpitaux sont utilisés de plus en plus comme une plateforme de communication pour tous ceux impliqués dans la gestion du malade, ceci en Suisse, pourtant, sans donner accès le plus souvent aux malades à l’information. La digitalisation des donnes est utilisée pour améliorer l’efficacité, mais peut aussi aider dans le processus décisionnel sur le plan clinique et administratif, et ainsi améliorer la qualité. Les progrès dans le domaine de l’intelligence artificielle offrent aussi de nouvelles possibilités en médecine. En plus d’assurer une protection fiable des données, garantir l’interopérabilité des systèmes utilisés est un challenge. L’échange de données sémantiquement structurées et lisibles à la machine est nécessaire pour un service en réseau et une recherche compétitive au niveau international.
