Clinical Management
Full ophthalmologic examination was performed. Largest basal tumor diameter and tumor thickness were measured by B-scan echography (I3, Innovative Imaging Inc., Sacramento, CA). If the diagnosis could not be established clinically, then tumor biopsy was performed: before September 2004, we used fine needle aspiration; after, we used a 25-gauge vitreous cutter.11 Tumor biopsy was not performed primarily for prognostication, except in one case at the patient’s insistence.
Before ocular treatment,
Results
Demographic and baseline characteristics are summarized in Table 1 (available at http://aaojournal.org). The 356 patients had a mean age of 61.9 years (range, 20.6–89.2). The tumors had a mean largest basal diameter of 15.4 mm (range, 6.2–23.6), and a mean thickness of 8.1 mm (range, 1.0–17.7).
Primary treatment consisted of enucleation (n = 302 cases; 84.8%); transscleral resection (n = 37; 10.4%); proton beam radiotherapy (n = 8; 2.2%); brachytherapy (n = 6; 1.7%); endoresection (n = 2; 0.6%);
Discussion
This study confirms that monosomy 3 is associated with a high rate of metastatic death in the first 5 years after treatment of uveal melanoma. Our study also demonstrates that estimation of excellent prognosis is improved by considering cytogenetic results together with largest basal tumor diameter and tumor cell type.
To our knowledge, this is the largest study yet reported on the prognostic significance of chromosome 3 and 8 abnormalities in uveal melanoma. The value of our investigation is
