      Diffuse subcortical brain damage of vascular origin (as evidenced on MRI by the presence of WMH) often favor the leakage of inflammatory cytokines which, in turn, disrupt the connections between the cortex and subcortical tissues. Such disruption contributes to abnormalities in balance and gait, hence increasing the risk of falls [2,3]. This effect is not seen with cSVD-related focal lesions.

      Last, atherosclerotic disease reduces blood flow to the brain and skeletal muscle. Particularly, white matter hyperintensities (ie, leukoaraiosis) reflect chronic blood flow reduction linked to cognitive deficits, gait disorders, and impaired balance leading to cognitive impairment and physical frailty and in turn to higher mortality.40 Taken together, the determination of the aforementioned factors may be useful for early detention of cognitive frailty, although further research is necessary to better understand the neurobiological basis of cognitive frailty and ultimately increase survival.
