A new anoscope for transanal surgery
2008, American Journal of SurgeryCitation Excerpt :Such points of rectal wall anastomosis are susceptible to leakage and potentially life-threatening infection, probably though perirectal bacterial seeding. Intramural abscess with fistula, subperitoneal pelvic cellulitis, rectal perforation with retropneumoperitoneum or pneumomediastinum, Fournier's gangrene, and rectovaginal fistula have all been described as a result of full rectal wall anastomosis13,15,16,18,19 and may require diverting colostomy with necrotic tissue debridement. We designed the new anoscope to overcome these problems and render transanal surgical procedures easier and safer.
Life threatening sepsis and mortality following stapled hemorrhoidopexy
2008, SurgeryCitation Excerpt :Incomplete rings (‘doughnuts’) of excised rectal tissue were noted in both cases. Clearly, placement of the pursestring suture is the key technical component of the procedure, and its misplacement (full thickness inclusion of the rectal wall, posterior vagina or peritoneum) is the main factor in these complications.30 Cheetham and Phillips note that along with the imprecision of pursestring height, “the depth of insertion of the pursestring is also imprecise, as we and others have seen because of muscle in the doughnut.”31
