7/7 patients were mycological cured and 5/7 clinical cured in the case study by Oliveira et al. (n = 7) at follow-up [20] and one open-label clinical study (n = 8 patients) demonstrated 6/8 mycological cure at follow-up [21]. All four cases treating onychomycosis with MAL-PDT obtained complete cure [7,22,23], but one case experienced recurrence at follow-up [22]. One case achieved mycological and clinical cure after urea pretreatment and nail abrasion followed by porphyrin-PDT treatment of onychomycosis [24].

Also, A review conducted on 5-aminolevulinic acid as photosensitizer showed penetration issues, making it not ideal for treatment [127]. Gilaberte Y et al. [128] used photodynamic therapy with 16% methylamino levulinate cream (MAL). 40% urea ointment was used to soften the nail plate and methylamino levulinate cream was applied.

In another study (n = 22), the mycologic cure rate was 100%, and the complete cure rates were 63.6% and 100% in patients with severe and mild to moderate onychomycosis, respectively.134 Across the 6 published PDT onychomycosis studies, negative microscopy or culture was found in 67% of patients (n = 58).133,134,142-145 Some disadvantages of PDT include the requirement for pretreatment with nail avulsion or urea, numerous sessions, and pain.133,134

There is some evidence of Herpes Simplex Virus infection (HSV) [110] and molluscum contagiosum [111] virus lesions responding to PDT treatment. Cutaneous mycoses have shown resolution in selected cases with good responses obtained in some series of onychomycosis [112]. It is also an acceptable therapeutic approach for refractory Malassezia folliculitis [113] or deep seated mycosis such as sporotrichosis [114].
