  • The roles of clinical audit and test sets in promoting the quality of breast screening: a scoping review

Sensitivity and specificity are two important evaluation measures for diagnostic tests, and it is important to understand the trade-off between them.31 Calculating sensitivity and specificity, nonetheless, is an inherent challenge in auditing breast cancer screening32 as they rely on classifying screened women based on actual outcomes, which includes correctly identifying false negatives. Such data are difficult to acquire in breast screening practice.

Again this is problematic in real life clinical situations as cancers could have been regarded as normal by screen readers but the accurate interpretation of these cases can only be determined after subsequent breast screenings. Instead the recall rate is often used as a surrogate of specificity; however, assessment of this clinical metric is affected by the prevalence of actual normal and abnormal cases in a particular population over a particular period of time.14 Thirdly, while the relationships between women who are recalled and have a final diagnosis of cancer or are reported as normal and truly have no cancer have been considered by the positive predictive value and negative predictive value, respectively,15 it is often useful to combine accuracy parameters such as sensitivity and specificity into a single figure.
