Abstract

The pattern of the metastases in intestinal and diffuse types of gastric carcinoma were compared in 77 autopsy cases. Differences in the extent of the dissemination, in the distribution of metastases, and in the type of secondary involvement were found. The diffuse type of carcinoma showed a wider dissemination than the intestinal type. The intestinal type of carcinoma involved the liver more frequently and more extensively. Peritoneal metastases, lymphatic permeation of the lungs, and Krukenberg tumors were more commonly found in cases of the diffuse type.

Regardless of their organ of origin, signet-ring carcinomas have a high likelihood of early, bilateral ovarian involvement in young women, potentially related to their affinity for ovarian stoma. Although disease may not always be evident clinically, nearly half of patients with gastric signet-ring carcinoma may have microscopic ovarian metastases, compared with 60% to 100% of patients with colorectal signet-ring carcinomas.19,22,23 Metastatic tumors are hypothesized to reach the ovaries via several key routes of spread: lymphatic, peritoneal, hematogenous, and/or direct extension.11,22

There are 2 distinct types of gastric adenocarcinoma including intestinal (well differentiated) and diffuse (undifferentiated). The diffuse type of gastric carcinoma is characterized by a wider dissemination of the tumor than the intestinal type, and is associated with a poorer survival rate than the intestinal type.5,6 The latter, however, involves the liver more frequently and extensively.5
