Acute traumatic coagulopathy is a well-defined phenomenon, with several pathophysiologic mechanisms similar to those seen in TBI, in addition to the potentially compounding effects of hemodilution, acidemia, and hypothermia.41-45 Patients with polytrauma have more profound disturbances to coagulation because of these mechanisms, as shown by the higher prevalence of hypocoagulability on ROTEM compared with isolated patients with TBI in the study by Tauber et al.26 The heterogeneity among these studies and the subsequent differences in results are important to inform future studies on this topic, especially with the selection of appropriate comparison groups, if the intent of the study is to guide management of TBI.26
