Abstract

The evolution of gonioscopy is reviewed, beginning with the first in vivo examination of the angle of the anterior chamber in 1867. The people and events responsible for the successful development of this important technique are discussed.

      The most versatile is the three-mirror Goldmann lens but it cannot be used to perform indentation gonioscopy. This technique is useful to determine whether the narrow angle has an appositional closure or a synechial one (Congdon et al., 1999; Dellaporta, 1975; Forbes, 1966; Friedman and He, 2008). Imaging the angle is crucial to identify the risk of PAC.

      Finally, comparisons of findings using the various techniques will be detailed. The current reference standard for evaluation of the ACA remains gonioscopy, a technique that was developed in the late 1800s by Trantas.16 Trantas physically indented the sclera in order to see angle structures using a direct ophthalmoscope, an ingenuous approach, but one which certainly distorted the structures he was viewing.95
