Aktueller Stand des Kolonkarzinomscreenings (Fokus auf Stuhltest)

Zusammenfassung

Die Abnahme des Kolonkarzinoms in Ländern mit hoher Beteiligung an Vorsorgeprogrammen zeigt, dass das Screening nützt. Mittels Koloskopie kann das Karzinom nicht nur früh entdeckt, sondern auch verhütet werden. Die Koloskopie ist aber nicht beliebt, invasiv, teuer und personenabhängig. Zunehmend sehen wir, dass ein vergleichbar gutes Bevölkerungsscreening mithilfe von repetitiv durchgeführten quantitativen immunchemischen Stuhltests auf Blut (qFIT) möglich ist; dies vor allem dank der weit besseren Akzeptanz von qFIT. Diese Stuhltests sind weniger belastend, benötigen weniger Manpower und sind kostengünstiger. Ein tiefer Cut-off-Wert verbessert die Sensitivität und verringert die Gefahr, ein Karzinom zu verpassen. Dies heißt aber auch: mehr positive Resultate und mehr Koloskopien. Die vorhandenen Ressourcen und Manpower beeinflussen deshalb die Wahl des lokalen Angebots an Vorsorgemethoden. Mittels zusätzlichen Nachweises von Tumor-DNS könnte die Sensitivität auf Karzinome weiter erhöht werden, allerdings auf Kosten vieler falsch-positiver Resultate. Möglicherweise gehört die Zukunft dem Nachweis alterierter Karzinom-DNS im Blut.
