CT-gezielte transgastrische Biopsie eines Pankreaskarzinoms

Eine 78-jährige Patientin wurde mit schmerzlosem Ikterus stationär aufgenommen. Es zeigten sich im Labor Cholestaseparameter (Bilirubin gesamt 16,0 mg/dl, GGT 680 U/l, AP 598 U/l). Die Patientin wurde zu einer Kontrastmittel-verstärkten Multidetektor-Computertomographie (MDCT) zugewiesen, welche eine große Raumforderung im Pankreaskopf mit Stenosierung des D. choledochus und Cholangiektasie (Abb. 1) sowie auch eine ausgeprägte venöse Gefäßinfiltration (Venenkonfluens und V. mes. sup.) aufwies. Aufgrund des reduzierten Allgemeinzustands wurde von einer Resektion Abstand genommen, sondern die Durchführung einer palliativen Chemotherapie nach Beseitigung der Cholestase durch eine biliäre Drainage angestrebt. Da für die Durchführung der Chemotherapie eine histologische Sicherung der CT-Diagnose eines duktalen Adenokarzinoms notwendig war, wurde eine endosonographisch-gezielte Biopsie durchgeführt, welche allerdings kein konklusives histologisches Ergebnis (stromareiche Pankreasstanzzylinder mit sklerosierender Pankreatitis und spärlichen Karzinom-suspekten Epithelverbänden) erbrachte. Daraufhin wurde die Patientin zu einer CT-gezielten Biopsie des Pankreastumors zugewiesen.
