Prävalenz und Erkennen der Depression an nicht-psychiatrischen Krankenhausabteilungen

Zusammenfassung

Anliegen

Das Ziel der vorliegenden Studie ist der Vergleich der Prävalenz der Depression zwischen verschiedenen Krankenhausabteilungen. Weiters sollten verschiedene Methoden des Erkennens von Depressionen durch Nicht-Psychiater verglichen werden.

Methoden

993 Patienten von internen, chirurgischen, gynäkologischen und somatischen Rehabilitationsabteilungen wurden von Forschungspsychiatern mittels der Clinical Interview Schedule (CIS) untersucht. Zudem wurden die jeweiligen Ärzte der Stationen gebeten für jeden ihrer Patienten einen kurzen Fragebogen auszufüllen. Durch den Vergleich dieser Fragebögen mit den CIS-Ergebnissen sollte erfasst werden, ob die Stationsärzte eine psychische Krankheit erkennen. Zusätzlich wurden die routinemäßig vergebenen psychiatrischen Entlassungsdiagnosen erfasst.

Ergebnisse

Insgesamt litten 13,3 % der Stichprobe unter einer Depression. An gynäkologischen Stationen fanden sich nur 8,7 %, an internen 9,5 %, an chirurgischen Stationen 9,8 %, und an Rehabilitationsabteilungen waren es 24,2 %. Durch die Fragebogenerhebung konnte festgestellt werden, dass die nicht-psychiatrischen Ärzte der Stationen 45,7 % der depressiv erkrankten Patienten erkannten. Allerdings fand sich nur bei 21,0 % der Personen mit einer Depression eine psychiatrische Entlassungsdiagnose, was weniger als die Hälfte jener ist, die von den Ärzten als psychisch krank erkannt wurden.

Schlussfolgerungen

Depressionen sind sehr häufig an somatischen Krankenhausabteilungen, werden aber im klinischen Alltag häufig übersehen. Routinemäßig vergebene Entlassungsdiagnosen sagen nur wenig darüber aus, wie häufig Ärzte an nicht-psychiatrischen Krankenhausabteilungen eine psychische Erkrankung wirklich erkennen. Folglich sind Routinediagnosen zur Planung der Versorgung von psychischen Erkrankungen nur wenig geeignet.
