Zusammenfassung
Der Morbus Crohn gehört zu der Gruppe der chronisch-entzündlichen Darmerkrankungen. Obwohl eine Heilung der Erkrankung bislang nicht möglich ist, gelingt es heute bei vielen Patienten aufgrund der zunehmenden medikamentösen Therapieoptionen eine gute Lebensqualität zu erreichen. Die eigentliche Wende in der Therapie des Morbus Crohn trat mit der Einführung der Tumornekrosefaktor(TNF)-α-Antikörper um die Jahrtausendwende ein. Das Behandlungsspektrum wurde in den letzten 2 Jahren durch Integrinantikörper sowie einen Interleukin(IL)-12/IL-23-Antikörper ergänzt. Die Remissionsinduktion erfolgt allerdings klassischerweise weiterhin mithilfe von Steroiden. Falls diese nicht ausreichen, sollte in Abhängigkeit der Krankheitsaktivität ein klassisches Immunsuppressivum, oder aber primär eines der neuen schnell wirksamen Biologika hinzugenommen werden. Im Krankheitsverlauf stellt die steroidfreie Remissionserhaltung ein zentrales Therapieziel dar. Hierzu kommen Mono- und Kombinationstherapien unter Einbindung von Azathioprin, Methotrexat und Biologika zur Anwendung. Mit dieser Übersicht soll aufgezeigt werden, bei welchen Patienten welche Substanzen heute eingesetzt werden soll. Aktuell stehen nur wenige Prädiktoren zu Verfügung, die eine Aussage über den weiteren Krankheitsverlauf ermöglichen. Langfristig muss neben der Entwicklung weiterer Therapieoptionen daher das Ziel verfolgt werden, solche Prädiktoren zu identifizieren, die eine individuelle Therapiesteuerung ermöglichen.
Abstract
Crohn’s disease represents a subtype of inflammatory bowel diseases. Although a cure is not available yet, the medical options have expanded significantly over recent decades and enable a better life quality for a significant proportion of patients. The turning point was defined by the introduction of tumor necrosis factor-α antibodies. This spectrum has been further broadened over the last 2 years by adding the integrin-antagonist vedolizumab and the interleukin-12/23 antibody ustekinumab. Induction of remission is, however, traditionally possible with steroids. If these are not sufficient, a classic immunosuppressive agent or primarily one of the new fast-acting biologics should be used depending on disease activity. In the course of the disease, steroid-free remission is a central therapeutic target. Mono- and combination therapies are used, including azathioprine, methotrexate, and immunotherapeutic agents. This overview is to show which patients use which substances today. We currently have only limited predictive factors that allow us to direct our therapy. Thus, the long-term goal must be the identification of predictive markers that allow for individualized therapy.
