Summary
Social psychiatry is concerned with the impact of contextual factors in its broadest sense on mental illness. Among the main topics of social psychiatry are epidemiology, health care planning, social consequences of psychiatric disorders (e.g. disabilities, stigma) and psychosocial interventions (e.g. interventions for relatives, activation, case management). The effectiveness of such psychosocial interventions was confirmed by a variety of controlled studies. Overall, biological, psychological as well as social factors contribute to the development of psychiatric disorders. Thus, all unidimensional or monocausal theories of mental disorders must be considered as outdated. In clinical practice, neither biological nor psychological nor social interventions alone are sufficient to treat persons suffering from psychiatric disorders. In fact, the combination of all these interventions is needed. Social psychiatry as well as biological and psychological psychiatry are essential in clinical work, in research and teaching. Each of these parties is indispensable for a comprehensive care of persons with psychiatric disorders.
Zusammenfassung
Sozialpsychiatrie beschäftigt sich mit der Bedeutung von Umgebungsfaktoren in weitestem Sinn für seelische Krankheiten. Neben der Beschäftigung mit der Epidemiologie, der Versorgung und den sozialen Folgen psychischer Erkrankungen (z. B. Behinderungen, Stigma) gehören soziotherapeutische Interventionen (z. B. Angehörigenarbeit, Aktivierung, Case Management) zu den Themenschwerpunkten der Sozialpsychiatrie. Die Wirksamkeit derartiger soziotherapeutischer Methoden wurde mittlerweile in zahlreichen wissenschaftlichen Studien bestätigt. Nach heutigem Wissen können biologische, lebensgeschichtliche und soziale Faktoren zur Entstehung von Krankheiten beitragen. Jede eindimensionale oder monokausale Theorienbildung muss daher als überholt gelten. Im klinischen Alltag ist weder die alleinige Beeinflussung biologischer, noch psychologischer oder sozialer Faktoren für die Behandlung ausreichend. Sozialpsychiatrie hat daher – genauso wie die biologische und die psychologisch-psychotherapeutische Psychiatrie – in der klinischen Arbeit, in der Forschung und in der Lehre einen unverzichtbare Aufgabe, ohne die eine umfassende Behandlung und Betreuung psychisch Kranker nicht gelingen kann.
