Zusammenfassung

Ziel dieser systematischen Literaturübersicht ist die Zusammenstellung relevanter klinischer Studien zur medikamentösen Behandlung eines Delirs bei Palliativpatienten. Die Arbeit stützt sich auf eine in Medline (ab 1966) und Embase (ab 1974) durchgeführte Datenbankrecherche unter Einschluss der Veröffentlichungen bis einschließlich Juli 2012. Von 448 Treffern konnten 3 Studien in die Analyse eingeschlossen werden. Auf der Basis dieser Studienlage kann die medikamentöse Behandlung des Delirs bei Palliativpatienten mit dem Antipsychotikum Haloperidol empfohlen werden. Dies gilt mit einer schwächeren Evidenzlage auch für die beiden Antipsychotika Olanzapin und Aripiprazol. Von der alleinigen Delirbehandlung mit Lorazepam ist abzuraten. Der in den Studien beschriebene Wirksamkeitsnachweis der Antipsychotika entspricht v. a. den umfangreichen klinischen Erfahrungen und der Praxis in der Therapie mit Haloperidol. Zur Absicherung dieser Empfehlungen bedarf es jedoch weiterer, aussagekräftiger Studien.

Abstract

This systematic literature review aims to collect and analyse relevant clinical trials for the drug treatment of delirium in palliative care. The search was conducted including July 2012 in Medline (from 1966) and Embase (from 1974). The search retrieved 448 studies, of which 3 studies could be included in the analysis. Treatment with the antipsychotic drug haloperidol can be recommended, which is also true to a somewhat lower extent for the antipsychotics olanzapine and aripiprazole. Treatment with lorazepam only should be avoided. This literature analysis reflects the positive clinical experience, especially when using haloperidol. To confirm these recommendations, further substantial clinical studies are needed.
