Zusammenfassung
Die ankylosierende Spondylitis ist eine entzündliche rheumatische Erkrankung des Achsenskeletts, die mit Schmerzen und Bewegungseinschränkung im Bereich der Wirbelsäule einhergehen kann. Im Spätstadium der Erkrankung können eine komplette Verknöcherung der gesamten Wirbelsäule und ausgeprägte Deformitäten resultieren. Diese gehen nicht selten mit einer deutlichen Minderung der Lebensqualität und bei zunehmender Einschränkung des Blickfelds einem drohenden Verlust der Selbstständigkeit einher. Patienten mit ankylosierender Spondylitis weisen ein erhöhtes Risiko für Wirbelsäulenfrakturen auf. Dabei handelt es sich meist um komplexe Verletzungen mit hoher Morbidität und Mortalität, häufig liegen neurologische Defizite vor. Die konventionelle radiologische Diagnostik ist oft nicht wegweisend, die konservative Behandlung von Wirbelsäulenverletzungen unbefriedigend. Operative Verfahren sind die Therapie der ersten Wahl bei frakturierten sowie in Fehlstellung ankylosierten Wirbelsäulen. Die Korrekturspondylodese der Globalkyphose führt zu einer Wiederherstellung der Statik, Normalisierung der Blickachse und Verbesserung der Lebensqualität. Durch operative Stabilisierung mit ggf. Dekompression, bei relevanten Fehlstellungen auch mit Korrektur, lassen sich akute Frakturen und chronische Frakturfolgen suffizient therapieren. Die Langzeitergebnisse nach operativer Versorgung sind trotz relativ hoher Komplikationsrate gut bis sehr gut. Minimalinvasive Verfahren können die Komplikationsrate weiter senken. Eine dezidierte präoperative Planung ist essenziell, um den Besonderheiten der Wirbelsäule bei Patienten mit ankylosierender Spondylitis gerecht zu werden.
Abstract
Ankylosing spondylitis is an inflammatory rheumatic disease that is often associated with back pain and restricted spinal movement. In the later stages of the disease, complete ossification of the entire spine and severe deformity can occur, often resulting in a marked reduction in quality of life and an increased risk of loss of independence due to diminished visual field. Patients with ankylosing spondylitis are at greater risk of spinal fractures. These are generally complex fractures associated with high morbidity and mortality; in addition, neurological deficits are not unusual. Conventional radiological diagnosis is often insufficient to establish a diagnosis. Conservative treatment of fractures of the spine in this patient group is unsatisfactory. Surgical procedures, if necessary combined with decompression, are often the preferred treatment of choice in the fractured or malaligned ankylosed spine. Rebalancing of the sagittal profile with normalization of the visual axis and an improvement of quality of life is achieved through corrective osteotomies. Despite the high rate of complications, long-term results following spinal surgery in patients with ankylosing spondylitis are good. Minimally invasive surgery is appropriate for a further reduction in the complication rate. Meticulous preoperative planning is essential in the treatment of patients with ankylosing spondylitis.
