Zusammenfassung
Modulare Psychotherapie setzt an die Stelle störungsspezifischer Psychotherapieprogramme Module als eigenständige funktionelle Einheiten, die untereinander vielfältig kombiniert werden können. Angesichts der Heterogenität in der Symptomatik der Borderline-Persönlichkeitsstörung (BPS) und der hohen Komorbidität werden als Therapie-Targets Funktionsbeeinträchtigungen gewählt, die direkt an den subjektiv erlebten Problemen der Betroffenen ansetzen. Solche zentralen Funktionsbeeinträchtigungen bei der BPS sind wie folgt: Affektdysregulation, Impulsivität/Enthemmung, Überempfindlichkeit gegenüber sozialer Bedrohung und Zurückweisung sowie defizitäre Mentalisierungsfähigkeit. Diese Funktionsbeeinträchtigungen, die bei Menschen mit BPS individuell unterschiedlich ausgeprägt sind, führen zur Auswahl geeigneter Module, die auf Techniken zurückgreifen, wie sie Bestandteile validierter störungsspezifischer Programme darstellen. Es wird ein Beispiel für ein modulares Psychotherapieprogramm bei der BPS für das Gruppensetting vorgestellt.
Abstract
Modular psychotherapy replaces disorder-specific psychotherapy programs with modules as independent functional units that can be combined in a variety of ways. In view of the heterogeneity in the symptoms of borderline personality disorder (BPD) and the high degree of comorbidity, functional impairments are chosen as therapy targets that directly address the subjectively experienced problems of those affected. Such central functional impairments in BPD are as follows: affect dysregulation, impulsivity/disinhibition, hypersensitivity to social threat and rejection, and deficient mentalization capability. These functional impairments, which may individually vary among people with BPD, lead to the selection of suitable modules that consist of techniques that are part of validated, disorder-specific programs. An example of a modular psychotherapy program for patients with BPD for the group setting is presented.
