Periprosthetic femoral fractures: treament and results in 51 cases

Summary

Periprosthetic femoral fractures present the problem of fracture treatment and prosthetic stability. Various treatment options are recommended for managing these fractures. We treated 51 patients with 54 periprosthetic fractures between 1985 and 1995. Of these, 42 (78 %) fractures were treated with a plate, eight (14 %), with a revision prosthesis, two (4 %), with retrograde nailing, and two (4 %), conservatively due to poor medical condition. The choice of fracture management was dependent on the type and location of the fracture and on the stability of the prosthesis. A total of 50 fractures healed primarily. Complications included three cases of plate loosening, one further fracture, one deep infection, one broken bolt and one varus deformity. Four patients died in the postoperative period, an additional 19 were dead at the time of the evaluation. Fractures with a stable implant can be treated with plate fixation. Cases of a loose implant require revision. Distal fractures are stabilized with a plate or with a retrograde nail.

Zusammenfassung

Die Oberschenkelfraktur bei liegender Hüftendoprothese stellt einerseits ein Problem der Fraktur an sich, als auch der Stabilität der Prothese dar. Zur Behandlung werden eine Vielzahl an Verfahren angegeben. Wir analysierten 51 Patienten mit 54 periprothetischen Femurfrakturen, die zwischen 1985 und 1995 behandelt wurden; 42 (78 %) Frakturen wurden plattenosteosynthetisch, 8 (14 %) mit einer Revisionsprothese, 2 (4 %) mit einem retrograden Nagel und 2 (4 %) wegen des schlechten Allgemeinzustands konservativ behandelt. Die Wahl des Verfahrens wurde von der Frakturform und Lokalisation, sowie von der Stabilität der Prothese abhängig gemacht; 50 Frakturen heilten primär und ohne Komplikationen aus. Zu einem Ausriß der Platte kam es bei 3 Patienten, einmal frakturierte der Oberschenkel distal der Platte. Eine tiefe Infektion und ein Bolzenbruch sowie eine Varusfehlstellung nach retrograder Nagelung sind weiters als Komplikationen anzuführen; 4 Patienten verstarben postoperativ an internen Leiden, 19 weitere waren zum Zeitpunkt der Nachuntersuchung verstorben. Frakturen im Prothesenbereich können bei stabilem Sitz der Prothese gut mittels einer plattenosteosynthetischen Versorgung behandelt werden, bei Prothesenlockerung sollte diese ausgetauscht werden. Frakturen distal der Prothese sollten plattenosteosynthetisch versorgt, oder wenn sie im suprakondylären Bereich liegen, mit einem retrograden Nagel stabilisiert werden.
