Zusammenfassung
Der habituelle Husten stellt eine wichtige Differenzialdiagnose des persistierenden Hustens dar und ist die häufigste klinische Entität innerhalb der „dysfunktionellen respiratorischen Symptome“ (DRS). Es besteht ein typischer trachealer, bellender, röhrender und lauter Klangcharakter. Der habituelle Husten tritt nicht aus dem Schlaf heraus auf und ist auf Aufforderung oft willkürlich reproduzierbar. Er hat einen appellativen, demonstrativen Charakter und stört die Umgebung meist mehr als den Patienten selbst. Für die Therapie entscheidend sind das Stellen einer Positivdiagnose, das Erläutern der zugrunde liegenden Mechanismen und Maßnahmen zur „reassurance“, d. h. der Beruhigung des Patienten und seiner Familie.
Abstract
Habit cough is an important differential diagnosis of persistent coughing. It is the most common diagnosis among the clinical entities of reactive airways dysfunction syndrome. Habit cough shows a typical tracheal, barking or honking sound. Habit cough does not occur during sleep and is usually voluntarily reproducible on command. The cough has an appellative, demonstrative character and often disturbs the surroundings more than the patients themselves. The mainstays of the therapeutic regimen are making a positive diagnosis, explaining the underlying mechanisms as well as measures of reassurance for calming the patient, the family and the surroundings.
