Zusammenfassung

Somatoforme Störungen im Kindes- und Jugendalter sind häufig und schränken das Wohlbefinden und die Lebensqualität stark ein. Aufgrund der hohen subjektiven Beeinträchtigung kommt es zu einer Vielzahl von Arztbesuchen, die oft durch wiederholt negative Untersuchungsbefunde ohne therapeutische Konsequenzen zu Hilflosigkeit der Patienten und zu einem Vertrauensverlust gegenüber dem Arzt führen. Anhand der Klassifikationsschemata der ICD-10 („International Classification of Diseases and Related Health Problems, 10th revision“) und dem Ausblick auf das DSM-V („Diagnostic and Statistical Manual of Mental Disorders, 5th revision“) werden die Kriterien der somatoformen Störungen beschrieben, und ihr typisches klinisches Bild wird erläutert. Bei ihrer Entwicklung im Kindes- und Jugendalter spielen die Eltern eine besondere Rolle. Der Pädiater ist für die frühzeitige Erkennung und Therapieeinleitung von entscheidender Bedeutung. Als Früherkennungsinstrument wird das neu entwickelte SOMS-KJ (Screening für somatoforme Störungen des Kindes- und Jugendalters) vorgestellt. Die betroffenen Patienten werden überwiegend im Konsiliar- und Liaisondienst der Kinderklinik gesehen.

Abstract

Somatoform disorders in children and adolescents are common and can cause a significant reduction in wellbeing and quality of life. Considerable subjective impairment leads to a high number of doctors’ visits which, together with recurrently negative findings, often results in a sense of helplessness in the patient and a lack of trust towards the physician. The criteria for somatoform disorders are discussed on the basis of the International Classification of Diseases and Related Health Problems (ICD)-10 and taking the upcoming Diagnostic and Statistical Manual of Mental Disorders (DSM-V) into consideration. Parents play a particular role in the development of somatoform disorders in childhood and adolescence. The importance of the paediatrician’s role in early diagnosis and introduction of appropriate treatment is underlined. The newly developed Screening for Somatoform Disorders in Children and Adolescents (SOMS-CA) is presented. Patients with somatoform disorders are seen predominantly within consulting and liaison services of the paediatric clinic.
