Zusammenfassung

Beim vorliegenden Beitrag handelt es sich um eine Stellungnahme der österreichischen Gesellschaften für Hals-Nasen-Ohrenheilkunde, Kopf- und Halschirurgie sowie für Kinder- und Jugendheilkunde zur Mandeloperation bei Kindern. Es gibt im Wesentlichen 3 Hauptindikationen zur Mandelentfernung, eine Hyperplasie der Gaumenmandeln mit Luftwegsobstruktion, wiederholte schwere Infektionen der Gaumenmandeln oder der Verdacht auf einen bösartigen Tumor derselben. Dabei ist stets zu überlegen, ob eine Adenoidektomie und/oder eine Tonsillotomie statt einer Tonsillektomie ausreichen, grundsätzlich ist die Indikation zur Tonsillektomie umso strenger zu stellen, je jünger das Kind ist. Da nach Mandelentfernung ein offenes Wundbett verbleibt, besteht immer ein Nachblutungsrisiko, über das bereits präoperativ detailliert aufgeklärt werden muss. Das richtige Verhalten im Fall einer Nachblutung ist sehr wichtig und kann Leben retten, ein entsprechendes Formblatt sollte bei jeder Mandeloperation mitgegeben werden. Abstract

The present article discusses the statement issued by the Austrian Societies of Ear, Nose and Throat Medicine, Head and Neck Surgery and Pediatric and Juvenile Medicine on pediatric tonsillectomy. Based on Austrian data and relevant reports in the literature, there are three main indications for tonsillectomy: obstructive tonsillar hyperplasia, repeated acute tonsillitis or suspicion of malignant tumour. Consideration should always be given to whether adenoidectomy and/or tonsillectomy are sufficient; in general, the younger the child, the more rigorously the indication for tonsillectomy is to be established, due to the risk of secondary bleeding. Since an open wound remains following tonsillectomy, there is always the risk of secondary hemorrhage, which must be discussed and clarified in detail preoperatively. Appropriate behaviour in the event of secondary bleeding is of utmost importance and can save lives. The relevant form should be issued for each tonsillectomy.
