Zusammenfassung.

Anhand der Kasuistik einer 46jährigen Patientin werden 2 Formen des artefiziellen Ödemes, der Selbststauartefakt und das sog. ,,hysterische'' Ödem (Oedème bleu de Charcot) klinisch und diagnostisch dargestellt. Bei der Patientin konnte eine therapieresistente Schwellung des linken Beines, die im Anschluß an eine tiefe Beinvenenthrombose aufgetreten war, als Selbststauartefakt entlarvt werden. Anschließend entwickelte sie ein Ödem des linken Armes, das sie durch unbewegliches Herabhängenlassen dieser Extremität erzeugte. Auch am Arm bestand eine Vorschädigung als Folge einer operativen Plexus-brachialis-Verletzung. Die Fallbesprechung erlaubt folgende neue Ansätze: • Auch ein Extremitätenödem mit faßbarem organischem Befund schließt ein artefizielles Geschehen nicht aus. • Stauartefakt und ,,hysterisches'' Ödem können von ein und derselben Person ausgebildet werden. • Die kontinuierliche Zusammenarbeit zwischen Dermatologen und Psychiater (Liaisonpsychiatrie) ist unerläßlich, um eine effektive Betreuung dieser Patienten zu ermöglichen.

Abstract.

Our report describes a 46-year-old woman with self-inflicted oedema of the left leg and the left arm. She induced leg oedema by an elastic bandage tourniquet and arm oedema by holding her arm dependent and immobile. A preexisting disorder of the affected leg (postthrombotic syndrome) and of the affected arm (hypoesthesia subsequent to surgical injury of the plexus brachialis) delayed the diagnostic proceedings. Our case report shows that: • Confirmed presence of organic disease does not exclude limb oedema of self-inflicted origin. • Oedema resulting from a tourniquet and ,,hysterical oedema'' can be developed by one and the same patient. • Effective care of such patients is only possible if dermatologist and psychiatrist work together all the time (liaison psychiatry).
