Zusammenfassung

Die Inzidenz der beiden häufigsten Tumoren der Haut, das Basalzell- und das Plattenepithelkarzinom, hat in den letzten Jahren erheblich zugenommen. Jeder Dermatologe wird mit den verschiedensten klinischen und histologischen Varianten dieser Tumore regelmäßig konfrontiert und muss in der Lage sein, tumor- und patientenadaptiert einen geeigneten Therapieplan zu entwerfen. Nach wie vor gilt die operative Entfernung dieser beiden Tumore als Therapie der Wahl. In Problemlokalisationen, bei großen Tumoren und bei weiteren Risiken für Rezidive oder Metastasierung sollte die Aufarbeitung mit kontinuierlichen, 3-dimensionalen mikrographischen Techniken erfolgen. Für oberflächliche oder initiale Läsionen kommen auch andere Behandlungsmöglichkeiten wie die Strahlentherapie sowie lokal destruierende Verfahren wie die Kürettage, Kryotherapie, Lasertherapie und photodynamische Therapie sowie lokale medikamentöse Behandlungen mit Imiquimod und 5-Fluorouracil in Betracht.

Abstract

The incidence of the most common tumors of the skin, basal cell carcinoma and squamous cell carcinoma, has risen rapidly in recent years. Dermatologists see in their daily practice many different clinical and histological variants of these tumors. They must be able to develop therapeutic strategies adapted to the tumor and the patient. Surgical excision remains the standard treatment. Micrographic histological evaluation should be employed in difficult locations, for large tumors and when there is increased risk of recurrence or metastasis. For initial or superficial lesions, other approaches such as radiation therapy, as well as curettage, cryosurgery, laser therapy and photodynamic therapy can be employed. An additional option is topical treatment with imiquimod or 5-flourouracil.
