Neoadjuvant Radiochemotherapy and Radical Resection for Advanced Squamous Cell Carcinoma of the Oral Cavity

Background and Purpose:

Several multimodal strategies have been developed to treat patients with squamous cell carcinoma of the oral cavity. The advantages of preoperative radiochemotherapy are downstaging of the primary tumor, an increased resectability rate, and the elimination of micrometastases. After successful phase II trials, the following therapy regimen for resectable advanced oral carcinoma was applied.

Patients and Methods:

134 patients with resectable squamous cell carcinoma of the oral cavity stage II–IV received neoadjuvant radiochemotherapy consisting of 39.6 Gy in daily fractions of 1.8 Gy and concomitant carboplatin (70 mg/m2 days 1–5). Radical resection and neck dissection were carried out afterwards.

Results:

After a median follow-up of 73 months, 82 patients (61%) had died. 54 patients (40%) experienced locoregional relapses or distant metastases. The overall survival was 65% ± 4% after 2 years and 45% ± 4% after 5 years. Cox regression survival analysis identified tumor regression, extracapsular lymph node spread and resection state as prognostic factors. Side effects of grade 3–4 were rare.

Conclusion:

Neoadjuvant radiochemotherapy with subsequent radical surgery can be recommended as an effective and safe treatment for primary resectable advanced tumors of the oral cavity. Acute and long-term toxicities appear to be moderate.

Hintergrund und Ziel:

In der Therapie des Plattenepithelkarzioms der Mundhöhle wurden bisher verschiedenste multimodale Strategien entwickelt. Der Vorteil der präoperativen Radiochemotherapie liegt in der Verkleinerung des Primärtumors, in einer erhöhten Resektionsrate und in der Erfassung von Mikrometastasen. Nach erfolgreichen Phase-II-Studien wurde das folgende präoperative Therapieregime zur Behandlung primär operabler fortgeschrittener Mundhöhlenkarzinome angewandt.

Patienten und Methodik:

134 Patienten mit operablem Plattenepithelkarzinom der Mundhöhle im Stadium II–IV (Tabellen 1 und 2) erhielten eine neoadjuvante Radiochemotherapie, bestehend aus einer Bestrahlung mit 1,8 Gy bis zu einer Gesamtreferenzdosis von 39,6 Gy und einer simultanen Chemotherapie mit Carboplatin (70 mg/m2 Tage 1–5). Anschließend wurde eine radikale Resektion plus Neck-Dissection durchgeführt.

Ergebnisse:

Nach einer medianen Nachbeobachtungszeit von 73 Monaten waren 82 Patienten (61%) verstorben. Bei 54 Patienten (40%) kam es zu einem lokoregionären Rezidiv oder einer Fernmetastasierung. Das Gesamtüberleben lag nach 2 Jahren bei 65% ± 4% und nach 5 Jahren bei 45% ± 4% (Abbildung 1). In einer Cox-Regressionsanalyse konnten die Tumorregression, das lymphknotenkapselüberschreitende Wachstum und der Resektionsstatus als prognostische Faktoren (Abbildung 4) identifiziert werden. Schwere Therapienebenwirkungen Grad 3 und 4 waren selten.

Schlussfolgerung:

Die neoadjuvante Radiochemotherapie mit anschließender radikaler Resektion kann als effektive und sichere Behandlung bei primär operablen fortgeschrittenen Mundhöhlentumoren empfohlen werden (Tabelle 3). Akut- und Langzeittoxizität sind akzeptabel.
