A new radiologic measurement for the diagnosis of Haglund's deformity

Summary

The authors describe a new method of radiologic measurement of “Haglund's deformity”, based on a radiologic study of 31 feet operated for posterior heel pain and more especially for calcaneal tendinopathies related to deformity of the calcaneus, and on a series of 60 asymptomatic feet. This angular approach requires a lateral weight-bearing view and proved positive in 85% of symptomatic feet, with only 14% of false-positives in the control group. It thus proves markedly superior to previously described radiologic formulations, since it allows not only for the size and site of the deformity of the posterosuperior part of the calcaneus but also for the angle of verticalization of the latter.

A partir d'une étude radiologique portant sur 31 pieds opérés pour talalgie postérieure et plus particulièrement pour tendinopathies calcanéennes en rapport avec une déformation du calcanéus, et sur une série de 60 pieds asymptomatiques, les auteurs décrivent un nouveau mode de détermination radiologique de la « déformation de Haglund ». Nécessitant un cliché de profil en charge, cette approche angulaire s'avère positive dans 85 % des pieds symptomatiques et ne retrouve que 14 % de faux positifs dans l'ensemble témoin. Elle s'avère donc nettement supérieure aux constructions radiologiques antérieurement décrites, du fait de la prise en compte de l'importance et du siège de la déformation de la partie postéro-supérieure du calcanéus mais aussi de l'angle de verticalisation de ce dernier.
