Summary

After a short review of the history of vitreous surgery we describe a new instrument which was developed mainly for a pars-plana approach of vitrectomy. The instrument may also be used for extracapsular surgery of congenital and traumatic cataracts by an anterior approach. The intraocular part of the instrument is composed of two small tubes with an outer diameter of 1.25 mm. It is designed for aspiration, cutting and removal of lens and vitreous structures. Normal intraocular pressure is maintained by infusion of physiological saline solution. The infusion device, ending in a rim in between the outer and the inner tube, is based on a simple hydrostatic pressure principle. The suction is performed under microscope observation by the assistant with an ordinary plastic syringe. The new cutting mechanism works on a stripper-like principle. Lens or vitreous structures are aspirated into a slit on the inner oscillating tube. A variable number of oscillating movements causes the aspirated structures to pass under the sharp edge of the outer tube and they are cut. The oscillations are generated by a small solenoid and an electronic control device. The instrument has proved to be very efficient in technical and animal experiments and recently in the first vitrectomies on human eyes.

Zusammenfassung

Nach einer kurzen Übersicht über die Entwicklung der Glaskörperchirurgie beschreiben wir ein neues Instrument, das vorwiegend für die hintere, über eine Öffnung in der pars plana ciliaris durchgeführte Vitrektomie entwickelt wurde. Es eignet sich aber auch für die Entfernung von kongenitalen und traumatischen Katarakten über eine korneale Öffnung. Der aus zwei Röhrchen bestehende intraokulare Instrumententeil hat einen äußeren Durchmesser von 1,25 mm und dient den folgenden Funktionen: Ansaugen, Abschneiden und Absaugen von Linsen- und Glaskörperstrukturen, Infusion von physiologischer Kochsalzlösung zwecks Aufrechterhaltung eines normalen intraokularen Druckes. Das Infusionssystem endet in einer Rinne zwischen äußerem und innerem Röhrchen und wird allein durch ein hydrostatisches Druckgefälle gesteuert. Das Anund Absaugen besorgt der Assistent unter Mikroskopkontrolle mit einer Plastikspritze. Neu ist ein auf dem Stripperprinzip beruhender Schneidmechanismus. Die in einem Schlitz des inneren, vibrierenden Röhrchens aspirierten Linsen- oder Glaskörperstrukturen werden durch eine beliebige Zahl von Vibrationsbewegungen unter der scharfen Kante des äußeren Röhrchens durchgezogen und abgeschnitten. Die Vibrationsbewegungen des inneren Röhrchens werden von einem kleinen Hubmagnet-Motor in Verbindung mit einem elektronischen Steuergerät erzeugt. Das Instrument hat sich nicht nur in Modell- und Tierversuchen, sondern bereits bei einigen Vitrektomien am Menschenauge bestens bewährt.
