Langzeit-Verlaufsbeobachtung des Keratoconus bei Unilateralität

Zusammenfassung

Hintergrund

Beschreibung des klinischen Verlaufes bei Patienten mit unilateralem Keratoconus, die auf dem besseren Auge mit Brille und am stärker betroffenen Auge mit rigiden Kontaktlinsen versorgt wurden.

Methode

Retrospektiv wurde aus einer Gesamtzahl von 112 Keratoconus (KC)-Patienten bei 17 (15,8 %) Patienten ein unilateral stärkerer Ausprägungsgrad des KC festgestellt. Die Beobachtungszeit reichte von 1972 bis 2006. Die durchschnittliche Beobachtungszeit betrug 12,37 Jahre. Das Alter der Patienten betrug zu Beginn der Beobachtungszeit im Durchschnitt 21,75 Jahre und am Ende des Beobachtungszeitraumes 36,76 Jahre. 6 Patienten waren weiblichen und 11 männlichen Geschlechts. Die Diagnostik erfolgte mit Hilfe der Spaltlampe und der Messung der Hornhautradien mit dem Ophthalmometer mit Beurteilung der Regularität der Miren.

Ergebnisse

Aufgrund unserer Erfahrungen wurde bei allen Patienten mit unilateralem KC die Korrektur des Sehfehlers solange wie möglich mit Brille vorgenommen. Am stärker betroffenen Auge entwickelte sich jedoch zunehmend ein irregulärer Hornhautastigmatismus, der mit Brille nicht adäquat korrigiert werden konnte, sodass dieses Auge mit rigider CL versorgt wurde. Der Durchschnittsvisus am mit Brille korrigierten Auge betrug zu Beginn des Beobachtungszeitraumes 0,89 und am Ende der Beobachtungszeit 0,82.

Schlussfolgerung

Aus unserer Sicht halten wir es bei unilateralem KC für günstig, den Refraktionsfehler am forme fruste Auge so lange wie möglich mit Brille oder weicher CL zu korrigieren, weil die Progression des KC und des irregulären Astigmatismus durch rigide CL gefördert werden können. Nachdem die Progression des KC ab dem 35.–40. Lebensjahr erfahrungsgemäß sistiert, kann mit Brillenkorrektur des forme fruste Auges auf diesem dauerhaft gutes Sehvermögen erhalten bleiben.
Der hier dargestellte Verlauf der Erkrankung bezieht sich auf die Behandlungsstrategie vor Einführung der „Corneal Crosslinking-Therapie“. Aus heutiger Sicht wird in jedem Einzelfall individuell zu erwägen sein, ob eine Crosslinking-Therapie durchgeführt werden sollte, um eine Progression der Erkrankung zu verhindern.
