Korneale Sensibilität bei Trockenem Auge – eine longitudinale Pilotstudie

Hintergrund

Das Trockene Auge stellt eine hochprävalente, multifaktorielle Erkrankung dar, deren detaillierte Pathogenese nicht gänzlich geklärt ist. Die Hornhautsensibilität besitzt durch die afferente Induktion der Tränensekretion einen essentiellen Stellenwert in der Krankheitsentstehung. Um das derzeitige Wissen bezüglich der nervalen Ätiologie erweitern zu können, wurde als Ziel dieser Studie die Beobachtung der Hornhautsensibilität im Verlauf und die Herstellung eines Zusammenhangs zwischen den Werten der Hornhautsensibilität und das Trockene Auge beeinflussende Parameter definiert.

Material und Methoden

In dieser prospektiven, nicht randomisierten Studie wurden 28 Patienten vier Mal in einem Zeitraum von 3 Monaten untersucht. Die Hornhautsensibilität wurde mit dem Cochet-Bonnet Ästhesiometer gemessen. Die Tränenproduktion wurde mit dem Schirmer(I)test quantifiziert, der Zustand der Augenoberfläche und des Tränenfilms an der Spaltlampe mittels Tränenfilmaufrisszeit, Fluorescein-, Lissamingrünfärbung und Lipidinterferenz beschrieben. Ätiologie, Pathogenese und Therapie wurden anamnestisch erhoben. Die Patienten wurden in Subgruppen unterteilt (hormonell, immunologisch, nerval, toxisch, umweltbedingt). Die Messergebnisse von Ästhesiometrie und Sicca-Parameter wurden mittels Pearson Korrelation verglichen. Für die gruppenbezogenen Untersuchungen wurde der Welch’s Test angewendet.

Resultate

Die Mittelwerte der Hornhautsensibilität im Verlauf lagen bei 4,89, 4,93, 4,91 und 5,08 mm. Die Verlaufskurven der Hornhautsensibilität zeigten sowohl inter- als auch intraindividuell starke Schwankungen. Zwischen Hornhautsensibilität und Sicca-Score konnte eine negative Korrelation beobachtet werden (R − 0,37). Zwischen Schirmertest und Hornhautsensibilität konnte kein Zusammenhang beschrieben werden. Die höchste Hornhautsensibilität zeigte sich in der umweltbedingten Gruppe (5,39). Es bestand kein signifikanter Unterschied zwischen der Hornhautsensibilität in der nerval bedingten ätiolologischen Gruppe und der nicht-nervalen Gruppe (p = 0,76).

Schlussfolgerung

Die Resultate dieser Studie zeigen eine starke Variabilität der Hornhautsensibilität bei Sicca-Patienten. Dies wurde sowohl inter-, als auch intraindividuell im Verlauf der Erkrankung beobachtet. Dadurch kann die Hypothese variierender Kompensationsstadien im Laufe der Erkrankung unterstützt werden.
