Knochenqualität bei der Versorgung orthopädischer und traumatologischer Schultergelenkserkrankungen

Biologie als tragende Säule in der Therapie

Zusammenfassung

Die Knochendichte und Knochenqualität sind von zentraler Bedeutung im Rahmen von orthopädischen und traumatologischen Erkrankungen. Verletzungen im Bereich des Schultergelenks werden oft von einer reduzierten Knochenmikroarchitektur begleitet.
Bei Patienten mit chronischen Rotatorenmanschettenrupturen ist die Knochendichte im gesamten Humeruskopf im Vergleich zu Menschen mit einer intakten Rotatorenmanschette herabgesetzt. Ebenso ist bei proximalen Humeruskopffrakturen die Gefäßversorgung des Knochens in Mitleidenschaft gezogen, mit direkten Auswirkungen auf die Biologie des Knochens des Humeruskopfs, da das Risiko einer posttraumatischen Humeruskopfnekrose durch eine begleitende Gefäßverletzung signifikant erhöht ist.
Neben einer chirurgischen Versorgung ist die begleitende Therapie der Knochendichte ein entscheidender Faktor. Eine begleitende Therapie mit Bisphosphonaten oder Teriparatid, einem humanen rekombinanten Parathormon, bei der chirurgischen Versorgung von chronischen Rotatorenmanschettenrupturen kann das Ergebnis signifikant verbessern. Auch eine adjuvante Therapie mit antiosteoporotischen Medikamenten zeigt vielversprechende Ergebnisse im Rahmen der Frakturheilung. Die operative Versorgung orthopädischer und traumatologischer Erkrankungen des Schultergelenks ist stark von der Knochendichte und biologischen Knochenqualität abhängig. Eine begleitende Therapie mit antiosteoporotischen Medikamenten kann das operative Ergebnis signifikant verbessern.
