Klinische Ergebnisse, gesundheitsbezogene Lebensqualität und Kosten-Effektivität eines 6-monatigen Programms zur Lebensstilmodifikation bei Personen mit erhöhtem kardioväskularen Risiko in Niederösterreich

Zusammenfassung

Zielsetzung

Evaluation der Wirksamkeit und Kosten-Effektivität eines strukturierten 6-monatigen Bewegungs- und Ernährungsprogramms bei Personen mit erhöhtem Herz-Kreislaufrisiko.

Methodik

Studiendesign als prospektive, unkontrollierte pragmatische Interventionsstudie. Erwachsene mit erhöhtem kardiovaskulären Risiko, jedoch ohne manifeste Herz-Kreislauferkrankung wurden eingeschlossen. Auswertungen erfolgten im Intention-to-treat-(ITT) Ansatz (Last-Observation-Carried-Forward) und wurden durch gesundheitsökonomische Modellierungen mit inkrementeller Kosteneffektivitäts-Analyse ergänzt. Studienendpunkte waren Veränderungen der kardiovaskulären Risikofaktoren (Serumlipide, Blutdruck, Plasmaglukose, Gewicht, Bauchumfang, Body Mass Index), des Bewegungsverhaltens sowie der gesundheitsbezogenen Lebensqualität.

Ergebnisse

Insgesamt nahmen 356 Personen (70,5 % Frauen; mittleres Alter 48,9 Jahre; mittlerer BMI 32,4; Dropout-Rate 10,4 %) teil. Zu Programmende wurden signifikante und klinisch relevante Verbesserungen bei kardiovaskulären Risikofaktoren, Bewegungsverhalten und gesundheitsbezogener Lebensqualität beobachtet. Die durchschnittlichen inkrementellen Kosten pro gewonnenem Lebensjahr verhielten sich invers zur Höhe des kardiovaskulären Risikos der betrachteten Population und das Programm war mit € 22.474 kosteneffektiv für die ITT-Population.
