Kardiologische Rehabilitation in Europa – eine kritische Analyse

Zusammenfassung

Die kardiologische Rehabilitation ist eine evidenzbasierte Maßnahme zur Verbesserung der Prognose und der gesundheitsbezogenen Lebensqualität von Patienten nach einem kardiovaskulären Ereignis. Grundsätzlich werden kardiologische Rehabilitationsprogramme in ganz Europa angeboten. Trotzdem bestehen nach wie vor große Unterschiede in der Struktur, der Länge und des Inhalts der Rehabilitationsprogramme in den einzelnen Ländern, auch verursacht durch eine unterschiedliche nationale Gesetzgebung und Finanzierung. Das Fehlen internationaler Standards wirkt sich negativ auf die Qualität der Programme und deren Ergebnisse aus. Ein eklatantes therapeutisches Ungleichgewicht besteht zwischen den für eine Rehabilitation infrage kommenden Patienten und den tatsächlich in ein strukturiertes Programm aufgenommenen Rehabilitanden. Nur in drei Ländern Europas werden mehr als 50 % der für eine kardiologische Rehabilitation geeigneten Patienten auch tatsächlich in ein stationäres oder ambulantes Programm aufgenommen. Auffällig ist der mit etwa 25 % sehr niedrige Anteil an Frauen. Die Rehabilitationsprogramme in Europa sind auffallend heterogen. Dies erfordert Maßnahmen zu Standardisierung über „best evidence“ zu „best practice“. Das Quality of Care Continuum der kardiologischen Rehabilitation könnte dazu einen wertvollen Beitrag leisten.
