Intraoperative chirurgisch-anästhesiologische Probleme und deren Konsequenzen für die Chirurgie

Zusammenfassung

Während die Vorgehensweisen bei schweren intraoperativen Komplikationen weitgehend definiert sind, ergeben sich im Alltag weniger dramatische, aber nicht minder wichtige Probleme und Fragestellungen, die von anästhesiologischer und chirurgischer Seite aus zum Teil kontrovers diskutiert werden. Präoperative Anämie, Transfusionstherapie, Volumentherapie, Lagerung, Wärmeerhaltung und Relaxierung stehen täglich im Fokus der teils gegensätzlichen Interessen. Gute beiderseitige Kommunikation, gegenseitiges Verständnis und die Berücksichtigung der individuellen Patientensituation sind Voraussetzung eines prospektiven Problemmanagements. Generelles Ziel ist die intraoperative Risikominimierung und Senkung von Morbidität und Mortalität. Dies kann erreicht werden durch ein modernes Volumenmanagement, blutsparende Techniken und Aufrechterhaltung der Normothermie. Von chirurgischer Seite ist zudem eine optimale Übersicht im Rahmen minimal-invasiver Eingriffe zu fordern, auch durch komplexe Lagerungen sowie eine adäquate Relaxierung.
