Innovationen in der Arzneimitteltherapie des systemischen Lupus erythematodes

Zusammenfassung

Hintergrund

Der systemische Lupus erythematodes (SLE) ist eine heterogene Erkrankung mit komplexer Pathogenese. Bislang war die Auswahl an Therapeutika begrenzt.

Zielsetzung

In dieser Übersicht werden bekannte Therapien des SLE neu betrachtet und neue, zuletzt zugelassene sowie derzeit in der klinischen Erprobung befindliche Substanzen vorgestellt. Ziel ist es, die aktuelle Datenlage aus Phase-II- und Phase-III-Studien sowie europäische Empfehlungen zum Management des SLE abzubilden und einen Ausblick auf die Zukunft der Lupustherapie zu geben.

Datenlage

Da in der Lupustherapie das Erreichen einer Remission mit niedriger Steroiddosis immer weiter in den Vordergrund rückt, ist eine frühe, effektive immunsuppressive Therapie unabdingbar. Mit dem Typ-I-Interferon-Rezeptor-Antagonisten Anifrolumab ist erstmalig seit 2011 eine Therapie für den extrarenalen Lupus zugelassen worden. Für die Lupusnephritis stehen das bekannte Belimumab (Zulassung durch US Food and Drug Administration [FDA] und European Medicines Agency) und der Calcineurininhibitor Voclosporin (FDA) neu zur Verfügung. Darüber hinaus befindet sich eine Vielzahl an Substanzen in klinischer Erprobung. Vielversprechend sind hier beispielsweise der CD20-Inhibitor Obinutuzumab, Januskinaseinhibitoren sowie niedrig dosiertes Interleukin‑2.

Schlussfolgerung

Neue und innovative Therapiekonzepte finden Einzug in die Lupustherapie und am Horizont finden sich weitere vielversprechende Substanzen. Inwiefern diese jedoch das langfristige Outcome der Patienten verbessern, werden erst Langzeitdaten zeigen. Dennoch sind dies wichtige und dringend gebrauchte Fortschritte in der Therapie des SLE.
