Inkontinenz nach radikaler Prostatektomie

Zusammenfassung

Die radikale Prostatektomie ist der wichtigste Grund für die männliche Belastungsinkontinenz. Die Inzidenzrate wird mit zwischen 6 und 69 % angegeben. Die Diagnostik beinhaltet u. a. eine ausführliche Anamnese einschließlich Tumoranamnese, körperliche Untersuchung, Sonographie und Urethrozystoskopie. Nach der Diagnostik erfolgt in der Regel zunächst ein konservativer Therapieversuch mittels Lebensstilveränderung und Beckenbodentraining für 6–12 Monate nach der radikalen Prostatektomie. Bei Versagen der konservativen Therapie bzw. wenn es zu keiner signifikanten Besserung kommt, wird ein operatives Vorgehen empfohlen. Es gibt zahlreiche operative Optionen, allerdings sollte die Auswahl in erster Linie nach den Kontraindikationen, dem Patientenwunsch und der Erfahrung bzw. Expertise des Operateurs erfolgen.
