Hypothyreose und Hyponatriämie

Koinzidenz oder Kausalität?

Zusammenfassung

Aktuelle Leitlinien empfehlen, im Rahmen der Abklärung einer Hyponatriämie eine primäre Hypothyreose auszuschließen. Die Evidenz aus größeren Studien und damit die Bedeutung für den klinischen Alltag sind hierfür jedoch nicht eindeutig und durchaus kritisch zu hinterfragen.
Es gibt verschiedene Erklärungsmechanismen, wie die Schilddrüsenunterfunktion eine Hyponatriämie auslösen könnte. Ein Ansatz geht davon aus, dass der bei hypothyreoten Patienten erhöhte ADH-Spiegel über eine Wasserretention zu einer Hyponatriämie führen könnte. Ein weiterer Ansatz beruft sich auf eine reduzierte Herzfunktion, welche durch eine Schilddrüsenunterfunktion ausgelöst werden kann. Dies sowie mögliche Veränderungen in der Niere, insbesondere eine reduzierte glomeruläre Filtrationsrate, könnten eine Hyponatriämie erklären. Ob diese Mechanismen jedoch auch für den medizinischen Alltag von Relevanz sind bzw. eine Schilddrüsenunterfunktion alleine ausreicht, um eine klinisch relevante Hyponatriämie zu induzieren, ist unklar. Eine kürzlich von uns publizierte Studie an einem großen Patientenkollektiv mit Hyponatriämie konnte keinen wesentlichen Einfluss einer Hypothyreose zeigen. In Zusammenschau der vielen Studien zu dieser Thematik lässt sich folgern, dass eine Hypothyreose als alleiniges Krankheitsbild, wenn überhaupt, sehr selten für eine klinisch relevante Hyponatriämie verantwortlich sein dürfte. Bei Patienten mit einem verminderten Natriumspiegel gibt es viele alternative Ursachen, welche nicht übersehen werden sollten.
