Hypochondrie als Heavy User in der Organmedizin

Zusammenfassung

Die Hypochondrie ist nach ICD-10 und DSM-IV durch die beharrliche Beschäftigung mit der Möglichkeit, an einer oder zwei schweren und fortschreitenden körperlichen Krankheiten zu leiden, definiert. Die hypochondrischen Ängste können zu einer hohen Inanspruchnahme von Leistungen im Gesundheitssystem führen. Daten haben gezeigt, dass die Gruppe der „Heavy User“ einen überproportional hohen Anteil der stationären Aufnahmen, sowie der Kosten im Gesundheitssystem für sich beansprucht. Wir gehen davon aus, dass ein psychotherapeutischer Ansatz ausgehend von einem verhaltenstherapeutischen Modell in Kombination mit einer psychopharmakologischen Therapie zielführend ist. Anhand einer Falldarstellung präsentieren wir das „Heavy Using-Phänomen“ mit Inanspruchnahme vorwiegend stationär-neurologischer Versorgung. Einerseits möchten wir aus einer medizinischen Überlegung heraus auf mögliche Behandlungsfehler hinweisen, andererseits auf finanziell-volkswirtschaftliche Faktoren, die zu einer massiven Belastung des Gesundheitsbudgets führen aufmerksam machen.
