Harnblasenkarzinom: Leben nach Zystektomie mit Ileumconduit und Ersatzblase

Die Therapie der Wahl des muskelinfiltrativen oder transurethral nicht beherrschbaren Blasenkarzinoms ist die radikale Zystektomie. Kontinente Lösungen orthotop sind die Ersatzblase an die Harnröhre oder heterotop an den Nabel mit Selbstkatheterismus. Die kognitiven und emotionalen Funktionen sowie das soziale Wohlbefinden scheinen mit einer Ersatzblase besser zu sein als mit einem Ileumconduit als Harnableitung. Die Wahl der Harnableitung muss aber immer individuell gewählt und der onkologischen Gesamtsituation angepasst werden. Die vollständig laparoskopisch roboterassistierte Zystektomie mit intrakorporaler Harnableitung ist technisch heute möglich und vielversprechend bei vergleichbarem Outcome, kleinerem Blutverlust und kürzerer Hospitalisierungsdauer. …
