Gerinnungsabklärung und antithrombotische Therapie bei gastroenterologischen Endoskopien

Zusammenfassung

Bei Patienten ohne antithrombotische Medikamente (orale Antikoagulanzien und Thrombozytenaggregations-Hemmer) kann eine Blutungsanamnese eine relevante Gerinnungsstörung entdecken. Eine routinemässige Bestimmung von INR und Thrombozyten vor Endoskopien ist nicht nötig. Vor diagnostischen Gastroskopien mit allfälligen Biopsien müssen antithrombotische Medikamente nicht gestoppt werden. Vor diagnostischen Koloskopien einschliesslich Screening-Koloskopien können die antithrombotischen Medikamente in den meisten Fällen belassen werden, da in 50 % der Fälle keine Polypen gefunden werden und die meisten Polypen klein (<10 mm) und direkt resezierbar sind. Einzig die direkt wirksamen oralen Antikoagulanzien (DOAK) würde ich vorausgehend routinemässig pausieren, weil dies einfach und mit wenig Risiko machbar ist. Anders als bei der koloskopischen Polypektomie gilt für andere endoskopische Eingriffe mit erhöhtem Blutungsrisiko weiterhin, dass die antithrombotischen Medikamente, mit Ausnahme der Azetylsalizylsäure (ASS), vorher pausiert werden sollten.
