Entwicklung eines Frühwarnsystems für kritisch Kranke im allgemeinstationären Bereich

Zusammenfassung

Hintergrund

Das Wiener Krankenhaus Hietzing mit Neurologischen Zentrum Rosenhügel (KHR) setzt sich seit 2014 mit der Entwicklung eines Frühwarnsystems für kritisch Kranke im allgemeinstationären Bereich auseinander. Hierzu werden physiologische Parameter der Betroffenen erhoben und diese, je nach Abweichung von der Norm, einem numerischen Wert zugewiesen. Die Summation der Einzelergebnisse ergibt den sogenannten Frühwarnscore, der je nach Risikoeinstufung eine Kaskade von klinischen Maßnahmen einleitet.

Zielsetzung

Vereinheitlichung eines hausweiten Vorgehens zur Risikoeinschätzung, Notfallalarmierung und der darauf folgenden klinischen Reaktion bei bedrohlichem Gesundheitszustand.

Methode

Konzipierung auf Grundlage internationaler und nationaler Empirie im Rahmen eines Projektes mit anschließender Testung und erfahrungsbasierter Weiterentwicklung.

Ergebnisse

Die Entwicklung eines Frühwarnsystems benötigt neben Know-How, Zeit bzw. praktische Erfahrung und ist mit einem Mehraufwand für das Personal der Pilotabteilungen verbunden. Die Praxis ist aufgrund der gesetzlichen Neuaufstellung der Zusammenarbeit zwischen Medizin und Pflege bzw. dem neuen Arbeitsgesetz des ärztlichen Personals gefordert, die ohnehin knapp erlebten Ressourcen effizient einzusetzen. Diese Voraussetzungen belasten den Entwicklungsprozess eines adäquaten Frühwarnsystems.

Schlussfolgerung

Die Organisation ist gefordert, innovative Antworten und Lösungen zu finden, die auf die Sicherheit der Patientinnen und Patienten fokussieren und die Belastbarkeit der Mitarbeiterinnen und Mitarbeiter nicht aus dem Blick verlieren lassen.
